I'm playing Insanity on my second time right now and it's about vet level first play at early stages at about 26+ and weapons so far(29 atm before I went to sleep,) things don't really get you that bad when you have geth shield (I went for 75% and 10% dmg of course,) and cloak since I'm an Infiltrator with the sniper from the collector ship and maxed out cryo ammo >=D.
