The 256 players comes from the game's Domination game mode. Things are chaotic, but don't expect to have all 255 other players on the screen. It's a huge map where you and the squads near you are trying to take the objectives on your side of the map while the other squads are taking their objectives on their side of the map. The idea is that the action starts out smaller scale, but as the attackers push the defenders back, things will get even more chaotic as the large numbers of players converge near the center of the map where the final objectives are.
