Pretty simple solution: you walk or run in place. The camera would have no difficulty recognizing what the player was doing. It would also recognize when a player turns left or right in place.
Nintendo already has a walk/run "simulator" in Wii Fit, only it requires the use of the accelerometer to measure steps like most pedometers. But it works, even if it isn't really a "game" from the standpoint of being entertaining and fun.
Personally, I don't see many developers requiring the player to constantly walk or god forbid, run throughout a Natal based adventure or action game though, so it's probably not even an issue unless the developer wants to make another fitness running simulator.
