Hmm. See, self mutilation implies that the guy is an M. So what he's doing is actually giving him pleasure. A true sadist will like to cause the M pain and suffering, so even if it may be things normal people will find painful, hurting an M is just counter productive. I like to think of it as the paradox of S&M. The S likes to hurt people, but Ms receive pleasure instead of pain.
