if it's like France it comes in the form of taxes companies have to pay based on employees salaries so it comes to the same in the end and has to be counted...( someone pays for that healthcare in the end).

 

It is public and voluntary (you can choose not to have an insurance). The cost (for me, in '07-'08) was about 30$ per month.

It might be worth considering that salaries for "specialists" of any kind is generally lower in Japan than in the U.S. Of course, I don't know if that applies to PD, but I'd think it does.
