There's actually been a few lens issues with the PS1. Some burnt out prematurely, others would get off alignment, or the worst one, was that the lens would come up too far and scratch the disk. I did have one in that last category, and fortunately, caught it before it rendered the disk unreadable. (Hearing a scratching sound from the disk drive is never a good thing.)
