Actually you can steal it from one of the official password cards, I'll see what it is and then post it in here afterwards.
The Cosmos Reports are obteined once you finish Shade Impulse chapter 4 with it's respective Cosmos characters. As for Chaos reports you have to use the respective Chaos character in the Duel Colosseum and fight it's respective Cosmos counterpart as a Boss Card. Any DC will do.
