Because it is your 3rd time I am guessing it worked the previous 2 times, that doesn't point to any initial problems with the disc. The disc was obviously OK at that time.
I am no expert but I have never heard of a CD, DVD or BR disc that stopped working properly without any physical damage to the disc or didn't require cleaning.
