How long did this occur after you purchased the unit(s)? About a year

When was the unit purchased (year/month)? Hmmm... Early 2006, not sure what month.

Which 360 model was involved? Launch model (Zephyr?)

Did you watch DVD movies on your 360?  How often? Very rarely

Did you watch streaming video on your 360?  How often? Very rarely

How often did you play games on your 360? 10-12 hours a week back then

Was your 360 on a power strip? Yes  A UPS? No  Did you switch your 360 off by switching off the power strip/UPS? No

How many other electronic devices were plugged into the same wall circuit? TV, Surround Sound, Subwoofer, so 3 others

Was the 360 positioned vertically or horizontally? Horizontally

Was the 360 operated on the floor? No      If so, what kind of surface (carpet, wood, tile, etc.)?

Was the 360 moved to other areas of the house on a regular basis? No  Was it transported to other homes? Very Rarely

Was the environment the 360 operated in dusty? Possibly a little

Did anyone smoke on a regular basis while the 360 was operating? No

Was the 360 operated on a shelf or in a cabinet? Shelf   Is the shelf/cabinet enclosed (glass doors, etc.)? No  Which shelf was the 360 on?  Top What other electronic devices shared the shelf unit/cabinet? just the TV (top shelf, plenty of open space)

Which of the following add-ons were attached?   HD-DVD?  Yes   WiFi?  No  Hard Drive? Yes   Decorative face-plate? Yes  Decorative body-panels? No

Was an add-on cooling unit attached? Yes  In-line with power supply?  USB powered? usb powered Pelican fan underneat it but still had 360 horizontally so the air was blowing on the bottom of the 360, not through the intakes.
