Tetris and Civilization (any game in either series). Nothing else has even remotely come close in my personal gaming experience.
I'm a bit surprised by some of the games listed here; they may be very well made, but "replayability" is a different subject. I mean, I love the Final Fantasy games, but they don't have much replay value (at least, the highly linear recent ones don't). The story is still the same each time, after all...
