I think they are popped bones sticking up out of the skin. They look like an aproximation of the ribcage and forearm.

This play mechanic is pretty fun and familiar, you play as a super hero (sane) who has to take down even stronger (insane) beasts. I think its used kinda a lot in Anime especially.

Well then maybe there will be tenticular sex.
