Yeah, I would definitely buy Knothole Island first. If you play it later it just feels tacked on, plus some of the rewards you unlock are hilarious and should be enjoyed for a large part of the game, not just however long you decide to run around once you're done. On top of that, it is a lot easier to unlock the rewards in the middle of a normal playthrough than to go back and get the stuff you need.
