What do you mean by a hole though? Is it dark? Can you poke your actual kneecap or something? 
edit: Like Epoch stated though, the main thing is if you notice the tissue around the actual bite getting dark. That's not a good sign. Now if by hole you mean the skin is decaying or something...go to a doc asap.
