Lol that R2 grenade thing threw me too.
The very first active combat thing I did was to blow myself up
I haven't had any game stopping bugs but I did notice something strange.
When I finished my first playthrough then redid some missions, the sound used to cut out every few seconds during a cutcene.
This stopped and became normal again when I started the campaign acain from the biginning.
I haven't patched the game yet though, so maybe my issue has been fixed.
