I think I've mentioned these games before, but the hardest games I've played (and not beaten, by the way) are Bubble Bobble and The Hitchhiker's Guide To The Galaxy. I'd also add that the original Civilization is pretty damn hard when you play it on the hardest level with the max amount of enemies.

Hitchhikers Guide to Galaxy isn't hard, it's bullshit.

Forgetting to get one item in the beginning of the game and then not needing it until the very end and if you didn't get it in the very beginning there's no way to win is the very definition of bullshit.

Well, that too :) But also, as a testament to it's difficulty I'd wager that every single player died at least a few times before even getting out of Arthur's bedroom in the beginning.

On another note, I just remembered Xenon 2: Megablast which would be among the best and most difficult shmups, at least if you don't have some kind of autofire cheat. In fact, I'd say that it's impossible to beat without one. And what happens when you beat it? Yep, that's right, you start again from the beginning only this time even the smallest enemies take twice as many hits to kill. Pure insanity, I say.
