There are three main problems.....
1) There government is fairly unstable, at times it was suspected the young people would overthrow the old guard, and there is still a chance of that happening to Iran going to a more democratic type of process as it goes on and on, as there are still lots of protests.
As such, during a revolution.... where the state is fighting for it's lives.... who's watching the missles? This is a problem even with russia today, though thankfully do to the USSR collapse being handled diplomatically rather then just falling apart, a lot of the problems were handled ok.
It's the same reason it's a huge problem that Pakistan has nuclear weapons. Only thing is... once someone has nuclear weapons, you can't take them away.
2) Iranian rhetoric seems to indicate that they don't understand the meaning... or don't care at least about "Mutual Destruction."
During the cold war, The United States and Russia came close to nuking each other many times. With less stabe and responsible leaders, the chances of this happening rises exponentially. It's possible they may shoot a couple of nuclear missles at Israel in hopes to wipe it out before retaliation... expecting the rest of the world to sit back and not do anything. Which is possible.
It's basically letting someone have a loaded gun who saying "I don't care if i get shot so long as I can shoot that guy.
3) Or just use nuclear weapons as a threat to other nations like Europe and the United States while invading other Middle Eastern Countires.... as Iran I believe still supports the "Greater Iran" notion that it's land should encompass "Greater Iran".
