I have heard of this several times in my life and a common thread across all of them seems to be that the Woman really doesn't want to be pregnant ... Every warning sign that would caused most women to go to the doctor (or at least to the pharmacy) to get a pregnancy test is ignored and explained away.
I wouldn't be that surprised if the pregnant woman was in a similar mind-state to the people who have $500,000 in credit card debt and are shocked when they're told the real value on some of those TV shows ... "$500,000? I thought it was closer to $25,000?"
