The PS2 cover was different from the Gamecube cover in RE4... then again they didn't release at the same time.  Who knows. 

 

 

Probably not, since the PS2 version was a port ages after the Gamcube version that was a different case. Same with the Wii version.

 

Even so, I hope it is different because I don't like the cover too much.
