As far as I know they had to censor it because it got the AO-Rating in the US, so big retailers like Wal-Mart wouldn't have putted it in their stacks. Also Nintendo and Sony don't want AO-Games on their Systems. So they censored it until they got a M-Rating (And they censored a lot to get one, so I doubt it was only the Pen-thingy, what I've heard a lot of executions and blood left the original to get the M). The BBFC still rejected to give it a Rating again (same as banned) so Rockstar went in appeal an won. So it got released here on 31.10.2008
