Snake survived years of attempted murders by one of the most fearsome, efficient killers on the planet... lung cancer.
Even if MC's suit could filter out the smoke from Snake's cigarette, and he didn't die from asphyxiation, Snake would still manage a smokescreen from it and get MC while he was blind, or lock a heat-seeking missile on MC using it, and it would all be over.
