We used to call it "Street Fighter thumb" back when Street Fighter II was released on the SNES. Other games had the same effect, but none punished the thumb like SFII on the SNES did.
I only occasionally get it now with the DS. Metroid Prime Hunters and Mario Kart DS were the two most recent that caused it. I think it has a lot to do with the small d-pad and how cramped your hand can be when using the DS.
