I'm very familiar with Nintendo thumb.
- The worst Nintendo thumb by far is "Gameboy Thumb" from the original grey brick.
- I get DS and PSP thumb frequently.
It's painful and it makes it hard to move your thumb at the first joint. Swells up sometimes. I just play through the pain if it's a good game. Those mighty endorphins released by sweet-sweet gameplay make the pain go away (until I finish playing).
