Motorstorm: Arctic Edge - not only did it go from PS3 down to PS2, people prefer to not even acknowledge that it exists

Tales of Symphonia: Dawn of the New World - worst lead character in a game ever. I liked the original, this one is worse in pretty much every way.

I can't really think of any more that were really bad without the 1st game being really bad too (like Deca Sports 2)

I will say FF13 as well if it counts.

As a true sequel I was most disappointed by L4D2. I played each level a couple of times with my friends and we all stopped after that. The original we kept going back to, the magic was just missing I guess.

Gears 2 : the online was completely broken.

Motorstorm Artic Edge is not a followup at all... It's a PSP / PS2 spinoff made by another team of devs. That's why it "went back to PSP" if it makes sense at all (it doesn't).

Motorstorm Pacific Rift is Motorstorm 2, it's on PS3 and it's far better than the first one.

Resident Evil 5 and FFXIII. Great games, bad sequels.

Sonic & the Black Knight (I suppose the "spiritual" successor to Sonic & the Secret Rings) was okay... it shined in some parts and failed to impress in others. I wish they would have just expanded on the WiiMote controllers from Secret Rings instead of giving him a sword.
