NEET and Hikikomori are not, actually, synonymous. NEET stands for Not in Education, Employment, or Training. It's not a purely Japanese term, just one adopted by the Japanese as they've had a recent problem with too many of them in their country. Many people can be NEETs, and it's one of the stereotypes of gamers (the 30 year old man in his mother's basement playing WoW).
A hikikomori is someone with social withdrawal; a shut-in, if you will. They isolate themselves completely from the outside world and social interaction. Most of these people have various (undiagnosed) social and anxiety disorders such as generalized anxiety disorder. A hikikmori often is a NEET, because their condition prevents them from easily acquiring any sort of employment, but there are a lot of NEETs who are not hikikomori.
