Why is this one picked on so much when in Assassin's Creed you ruthlessly kill people, in Uncharted 2 you kill hundreds of people, in Final Fantasy VI a whole nation is poisoned because one guy is impatient, in Mass Effect you can choose to let hundreds of people die because you don't want to do sidequests, in GTA you can run around killing innocent people all you want, and so on.

In Assassin's Creed, your job is to kill evil people to preserve peace. The creed of the assassin is to kill one person to save thousands, and the act is a act of justice. In Final Fantasy VI, is the death of a nation something you do, or a plot point? In Mass Effect, you are forced to make tough moral choices. It goes with the game. And in GTA, you can be a jerk if you want, it fits that game. One can just do the missions though. Mind you, it is the mindless killing of innocent people I do find makes me not like the GTA series.
As for MW2, early on, the mission involves going with terrorists who gun down civilians. I couldn't even pull the trigger on civilians myself when doing it (I did take out the police though). It is understandable why it is optional, and would be offensive to people.
