Because we actually want everybody to have clean assholes, so that's not as insulting.  But the majority of people prefer filthy mucky icky vajayjays, and calling somebody a tool to clean one implies that they prefer them clean, which means they're a sexual deviant, and possibly gay.

Are they buying douchebags though?  They rarely come in bags these days.  I only see advertisements for actual bags of douche in old magazines from the 70's.  I should find some again and put them up on my wall or something.  They're rad.

I dunno. Its just these boxes that say DOUCHE in big letters on them, so they tend to stand out a bit. I've never seen the contents of one.

I've worked at two different grocery stores and they both had douche.
