When watching a video, press triangle, then click on change icon, it will ask you if you want to start the 15sec timer and you agree to it. Whatever happens in the next 15secs will be your icon.
But at the end of game videos off the PSN they always show the games name then say PS3 and some other stuff. If you start the recording when the video is on the name it will show that then the other screen with PS3 on it. Your icon video will only be a couple of seconds then if you don't want a long one.
