Your MAIN final ending really matters on the decisions you make after Kaer Morrhen or whatever is unlocked, that's all I have to say. Besides that, every little decision you make for characters or whatnot will have a short cutscene throughout the game later on telling you what your action spawned.
I've seen the 3 main endings, but boy am I glad I got the ending I wanted. Choose wisely!
