Project hammer got canned for that very reason. It happens alot, probably more than we know because we don't hear about the games that get scrapped.

They don't do this because most of the cost is in development and not actually producing/packaging/shipping a game. If most of the cost is already incurred, they hope that they can just ship it and get some money back on it at least rather than just eating the whole cost. OTOH, for certain games (especially with an established series), this has to be weighed against damaging the brand name. I think that's why something like Starcraft: Ghost never saw the light of day.

This is the reason. By the time a developer realizes a game is just going to suck it is too late. The decision becomes how much would it cost to finish versus how much we can make. Essentially the games are shipped hoping to recoup the cost of making and shipping the game itself.
games get canned all the time, many bad games come out because they want to make some money. For companies like nintendo and blizzard their reputation of putting out good games is very important and they will scrap games if they are bad. If you're EA, then you'll probably release it anyway.
