From what I've been able to gather, this seems to be mostly related to fatigue (burnout), but it's hard to say for sure since Uematsu himself has only really alluded to this fact rather than clearly stating what the issue is. I hope that's true still so it's nothing life threatening.

Then you could have just said they didn't want to be in incurable diseases that cause them so much pain anymore.

My first post was that I was expecting the best outcome, which would be quick relief and he being health or in case of incurable disease with excruciating pain and he not wanting to live anymore that he could have a peaceful pass away. You made a huge mountain of less than a mole.
