Kratos has 2 signature scars. One in the right eye and one in the stomach/abs.

Cory Barlow says, kratos starts living in Norse region after the events of gow3. You know there was no dead body. Pretty much this is a sequel.
I think the moat obvious "surprise/plot twist" is that you actually won't play as kratos for much more than what would serve as the tutorial level in this game.
Clearly evident in kratos snippet with all the "new beginning" thing, the fact that after certain tasks the kid did we see a message on the screen saying "knowledge gained" and Cory's own giveaway by out rightly saying "you play as kratos throughout the game".
So yeah, we are going to be playing as his son throughout. Obviously older but his son none the less.
It probably is a sequel, however i'm not too sure Kratos will be the main character in the game. He is clearly training the boy and at the end he says "now you are ready" and "this is a new beginning".
