The mirror part was just for your online avatar I assume as the Doctor never actually goes through with the surgery.
The kid is speculated to be a boss from the original MGS (Mantis), the fire guy looks like another boss (Volkin) although his ability is different some what as that boss was like a walking railgun using electricity to propel objects. It could be that the man on fire is not a person and just a manifestation of Mantis' Pyrokinesis but that's speculation right now.
