I got it a few days ago and played a few hours of it. It's pretty awesome. AAA+ title. I'm going to play some later today. And finally get to check out the online portion as it wouldn't let you activate your online account until today.

As with most games, I'm going to set in on the most difficult setting available for a new game, largely because I enjoy the challenge, and it usually makes me appreciate the gameplay mechanics more.

Same reason for me.  Hard is the hardest you can do on first playthrough, Survivor unlocks after that.
