But I feel like the plot twist might be something like this:

At some point early-ish in the game, Elly will lose someone close to her. Probably her girlfriend. Perhaps when we see someone getting shot in the new trailer while Elly is held down by some thugs.
Elly will then go into her "I'm going to kill every last one of them" phase, and we'll assume she is talking about her dead girlfriend. She'll team up with Joel to take down the enemy.
But then later on in the game it'll be revealed that Joel was killed a long time ago. Perhaps during a crucial moment where we think Joel is with her and can help her, but turns out he's actually not there, and can't help. And that the scene where Elly said she was going to "kill every last one of them" was actually a flashback that happened before her girlfriend got killed. And that she was referring to Joel's death.
This is probably too specific to be accurate, but something along those lines.

I thought they just haven't seen each other in a while...maybe she cut ties with him, because she found out, what really happend at the end of the first part.
It's an interesting theory, but i think he's alive. Mostly because he interacts physically with her in the trailer and there's bit where Elly sits on the passenger seat (and gets attacked) while presumably some else is driving. The second "evidence" is a far stretch, but i'd put my money on Joel.
