I didn't have the same problem, i saw my dead driver burning in the clearing in the middle of the town, marched in there like John Wayne and was promptly killed.
Second time i noticed the open door, the only one in the village incidently, so went in there which triggered the cut scene. Once you've got the shotgun it's just a matter of running and gunning a little or finding a nice safe wall to put your back to, only pulling out the shotty when it's absolutely necessary. I ended up on the roof and because all the guys can see you, you kill your quota pretty quickly.
Now that i think about it there are a couple of other spots where the trigger for progression may not be very obvious either but most of the time it's right in front of you.
